Thank you for choosing my clinic for your care which specializes in the evaluation and treatment of ADHD and related conditions in children, adolescents, and adults, using evidence-based strategies and individualized care plans.

 

How to Schedule Your Appointment:

1. Select an appointment type from the list.

2. Pick an available date and time.

3. Complete the brief questionnaire for required pre-visit clinical information.

4. Submit payment to confirm your appointment.

5. Check your email for confirmation and the link to Zoom will be included in the email.

6. Automatic reminders will be sent: 7-days, 48-hrs, & 1-hour before your appointment.

 

Fees & Payment Policy

• All visits are private pay only. Insurance is not accepted.

• Initial evaluation: not available until 2026

• Follow-up visits range from: $225-$300

• Payment on file is collected via Stripe at the time of scheduling.

• Patients have the option to pre-pay or will be charged after services are rendered

 

Office Policies for Scheduling

• Please cancel or reschedule at least 24 hours in advance to avoid a late cancellation fee.

• Late cancellations or no-shows may result in a full appointment charge.

For Parents Scheduling for a Minor:

• One parent is legally required to consent in most cases, unless a custody order requires joint consent.

• Please ensure the child is present for all evaluation appointments.

• You may request a joint parent–provider session, if both parents agree, prior to treatment.
